Local Church/Charge Conference Forms

Most of the forms linked on this page are distributed as paper copies from the District Superintendents to each church during the Summer, although some of the Districts are no longer doing that (in an effort to use less paper), so you'll find your forms here. The links below marked with an asterisk are to PDF versions that may be completed on a computer and then printed out. In addition, MS Word document versions have also been provided where appropriate if using the PDF versions doesn't provide enough flexibility. Some of the forms below are optional, so please refer to the instructions sent to each church for details. If you have questions about the forms, contact your District Office for clarification.
