Health & Welfare Survey Information
The Health and Welfare Conference Committee has created a survey that will help us gauge where we are succeeding and where we are lacking in our congregational caring and health ministries. This will allow us to create trainings and resource materials that are targeted to the needs of the local congregation so that we can prepare you to serve the many generations throughout this church conference.
If you have not received the survey via email and would like to participate, please download the survey form below, fill it out, save it, and email to health(at)desertsouthwestconference.org.
The Health & Welfare Committee will be presenting an educational opportunity to facilitate health-related parish networking, present enrichment and prevention programs and to address the needs found in the survey.
