DSC boards, committees, and agencies may submit requests for facilities in the 2012 Annual Conference in Glendale, AZ by using the form provided at the link below. An introductory letter with guidelines for the request process is also available at the link below. Please submit the completed form to the DSC Faciliites Coordinator, April Martinez no later than April 16th, 2012.
