Many of the forms linked on this page were previously distributed as paper copies from the District Superintendents, but in an effort to use less paper and save on postage, the forms are now being distributed digitally here on the DSC website. The links below marked with an asterisk are to PDF versions that should be completed on a computer and then printed out. Some of the forms below are optional, so please refer to the instructions sent to each church for details.
The best way to work with the forms below is to download them to your local computer, rather than opening them in your web browser. To download individual forms, "right-mouse-click" on the link for the desired form and then left-click on the "Save target as" or "Save link as" option on the menu that appears. You should be able to save them somewhere accessible on your computer, where they'll be easier to access in the future without needing to return to this website.
Note to Mac users: we have discovered that using the default "Preview" program on a Mac to open and fill out the PDFs below can result in problems, so we recommend using the free Adobe Reader program to open and work with the PDFs.
